About Fresh installation in cluster env

When company has fresh oracle application setup does it first have

Shared APPL_TOP on 2 nodes and then using the clustering concept move the form server and concurrent processing server to cluster node individually?

eg

1st node initially has Concurrent Processing server, Report server, Form Server and admin

2nd node initially has HTTP Server (Web Server)

Then other 4 new instance are setup 2 each in seperate clusters one set for Form Server and another set for Concurrent Processing Server...

Another thing what about load balancing in this situation? do we need two seperate load balancing servers? one for Form server cluster and another for Concurrent Processing Server?

Is that correct? I am just trying to understand how it is done. Please correct me if i am wrong...
