How to run custom reports from form menu in r12

Hi,

Im working on 11i to r12 upgrade. My forms are working fine. I want to run my report from forms menu(button press). My reports works fine in report builder and fetches output, but when i run in server, it shows http 404 page not found. Am i missed any set ups. I dont have a stand alone report server. Its an apl\plication server, both forms and reports together. i use executable rwservlet using web.show_document to run reports in web.
Please clarrify on running the reports in server?
