I have not done it myself but I have discussed with few. Since from business point of view all 3 have same powers , hence they will have same position in apps. They may have different jobs. So the approval will go to all of them.

During any implementation , the tem does testing and development on various instances , before going live. These are test and dev instances. Once we go live then the instance is called as prod - all transactions are based on PROD.
