Issue has been resolved by removing the "Manual" sequence assignment, while there exists "Automatic" sequence assignment.

The error started from 26-Feb-2009.
There is no change from front-end or back-end.
On TEST environment, same transaction successfully saved.
There have 5 transactions already saved on the same sequence successfully.
