I am looking for some document which can help me plan the starategy for migration.

You can also go through "Oracle Applications Upgrade Guide: Release 11i to Release 12.1.1" document (E14010-01)" which can be downloaded from the following URL.

If you are having below 11.5.7, then you should first upgrade to 11.5.10 and then to R12, otherwise you can directly upgrade to R12.
