Error in forms

Hi,

I have created new application top ,I had developed a new form and register in the front end,but while running the form i am getting the error that Funtion name is not attached to the responsibility.

Check either function is attached to menu
by navigating to responsibility and get menu name and then query menu and attach your form (function name ) with menu
