ifweb60.exe - Application Error: The application failed to initialize

After upgrade 11 to 11i. That built on Oracle Forms/Reports on a server with Windows 2000 Server SP 4.Now, since a new application has been introduced, I have been experiencing problems that force me to reboot the server. I have been encountering the following message:

ifweb60.exe - Application Error: The application failed to initialize properly (0xc0000142).
