Can some one tell me the name of DFF as shown in the file.

If you have access to diagnostics, then you can find the DFF name using the below method.
Go to Help (from tool bar) --> Diagnostics --> Examine (Enter Password, if required) --> Select Block Name as "$DESCRIPTIVE_FLEXFIELD$"
Then select appropriate DFF field. In the value field, you will get the application and the DFF Title
