Setup Benefit Contributions in HRMS?

How to setup Benefit Contribution, and populate Benefit Plan, and Benefit Contributions Coverage information?

I need to do this following example in the Prod, these example data get from Vision.

Name: Medical Option
Benefit Classification: Medical.
Primary Classification: Pre-Tax Deduction.
Carrier: prudential.
Period Type: Calendar Month.
* COBRA enabled.

Coverage: Employee only
Employer: 189.21
Employee: 62.74

So, how to populate these LOVs and how to setup Benefit Contributions in HRMS?
