When an issue is identified/reported in production instance, it will be assigned to the support team (inhouse/external).
The support team (consists of functional and technical consultants) will analyze the issue in DEV instance. There may be setup changes or code changes or sometimes just a clarification. In case of setup change/code change, the same will be done in the development instance and then testing will be done by the support team to ensure that the fix is correct.
If the setup/code fix is working fine in development instance, then the same will be copied to TEST instance and it will be communicated to the respective users. The users (who raised this issue) will test the issue in TEST instance and then give clearance to apply the fix in the production instance (if the issue is resolved)
The setup/code fix will then be copied to production instance as a individual patch or a group of patches (depends on the company policy).
All live data are entered in production instance. The development and Test instances are clone of the production instances.
