Is there any link or document which will describe about the R12 issues till R12.1.2 if possible

There is nothing of that sort available directly .. However if you are downloading 12.1.2, the documentation of the relevant version will state the list of bugs that has been fixed in the current version. You can consider that list of bugs as the issues fixed till 12.1.2 ... Also periodic critical patch updates will also come with similar list of issues fixed ..
